Cargo

The Insured must send to ASSA, within 30 days from the date of the loss or damage, the formal claim in writing, detailing the lost or damaged objects and the amount of the claim, proving his insurable interest and the occurrence of the loss or damage, including receipts, invoices, appraisals or other documents that demonstrate such interest and prove the amount claimed.

  • Original or Negotiable Copy of Commercial Invoice (Not photocopy).
  • Original or Negotiable Copy of Packing List (Not photocopy), indicating gross weight, net weight, individual numbering and contents of each package.
  • Original Ocean, Air or Inland Bill of Lading, as applicable.
  • Damage Report (Inspection Report), issued by recognized independent inspectors.
  • Original of the Note of Exceptions, issued by the last carrier or entity responsible for the carriage or custody of the cargo during transit.
  • Copy of the Letter of Notice of Claim to the Carrier (issuer of the Bill of Lading, Air Waybill or Waybill), with the acknowledgement of receipt reflecting the date, signature, and seal of the firm that received the notice, within the term established in the respective transport agreement.
  • Report of the insurance premium corresponding to the shipment subject of this loss, in order to activate the file that will allow us to provide the necessary assistance to formalize your claim properly.
    Letter or corresponding form, detailing the documentation submitted and calculations of the value of your claim.

If applicable, the following additional evidence or documents will be required to support your claim:

  • Laboratory Analysis
  • Destruction Records
  • Phytosanitary Examination
  • In-transit Container Temperature Charts or Logs
  • Quotes or Technical Evaluation

Ground Cargo

After the notice of loss, within 30 calendar days following the date of loss, the Insured shall submit to the Company the following documents:

  • Original or authenticated copy of the Police Report in case of a traffic accident, including a report from the National Fire Department in case of fire.
  • Statement or report made before the competent authorities in case of assault or robbery.
  • Copy of the vehicle’s ownership documents, in the case of vehicles of the insured named in the policy.
  • Originals (not photocopies) of the Commercial invoices confirm the total value of the shipment held by the transporting vehicle when initiating the transit.
  • Proof of all merchandise or goods whose delivery or sale had been made prior to the loss.
  • Location of the transporting vehicle and remains of the cargo for pertinent inspections.
  • Proof of salvage expenses incurred to mitigate losses.
  • Copy of the note or notice of claim against third parties responsible for the accident.

If applicable, the following additional evidence or documents will be required to support your claim:

  • Laboratory Analysis
  • Destruction Records
  • Phytosanitary Examination
  • Container Temperature Charts or Temperature Records during transit
  • Weights and dimensions cards
  • Quotes or Technical Evaluations

Electronic Equipment 

After notice of loss, within 30 calendar days following the date of loss, the Insured shall submit to the Company the following documents:

  • Keep the damaged parts and make them available for inspection by a representative or expert/expert assigned by the insurance company.
  • Expert report from qualified personnel or company to determine the origin/cause of the loss and the extent of the damage.
  • Original complaint filed before the Judicial Investigation Organism (OIJ) and copy of the ocular inspection of the OIJ, in case the loss or damage is due to theft.
  • Proforma invoice of the same or similar equipment to the damaged one, indicating the replacement value of the equipment.
  • Quotations or proforma invoices that support the value of the loss.

Fidelity

After the notice of loss, within 30 calendar days following the date of loss, the Insured shall submit to the Company the following documents:

  • Immediately report to the competent authorities the crime occurred (OIJ), in order to initiate the corresponding judicial investigation and submit a copy of the criminal accusation formulated against the suspected employee or employees, before the competent authority, with the seal of receipt and assigned summary number.
  • Reports of audits carried out at the time of the loss and the last audit carried out in the area where the accused personnel worked.
  • Submit documentation showing the amount of the loss, in accordance with generally accepted accounting practices.
  • Legal capacity of the general attorney of the company and legal identity card.
  • Photocopy of the identity card of the legal representative or attorney-in-fact.
  • Provide personal data of the accused (full name, age, marital status, identity card number, position held in the company, last salary earned, date of joining and leaving the company, personnel action, photo and any other data that may help to identify or locate him/her).

Fire

After the notice of loss, within 30 calendar days following the date of loss, the Insured Party shall submit to the Company the following documents:

  • Report or certification of the loss issued by The corresponding Authority (National Fire Department).
  • Detailed and exact inventory of the property destroyed or damaged and the amount of the loss in each case, not including any profit.
  • Labor estimates from one or several contractors and quotations for materials.
  • Photocopy of the last appraisal of the residence (not required).
  • Detailed list of any other insurance covering any part of the insured property.

Civil Liability

After the notice of loss, within 30 calendar days following the date of loss, the Insured shall submit to the Company the following documents:

  • In case of damage to the property of others, submit labor quotes from at least two suppliers or contractors.
  • Quotations of the materials required by the supplier or contractor.
  • In case of injuries to third parties, the general information of the injured party or parties must be provided in order to coordinate an evaluation by a doctor of the Insurance Company.
  • If there is a report or certification accrediting the person responsible for the accident, this certification must be sent to the corresponding authority.

Multi-Risk

The Insured must send to ASSA, within 30 days from the date of the loss or damage, the formal claim in writing, detailing the lost or damaged objects and the amount of the claim, proving his insurable interest and the occurrence of the loss or damage, including receipts, invoices, appraisals or other documents that demonstrate such interest and prove the amount claimed.

Theft

After the notice of loss, within 30 calendar days following the date of loss, the Insured Party shall submit to the Company the following documents:

  • Original complaint filed before the Judicial Investigation Organism (OIJ).
  • Copy of the ocular inspection by the OIJ.
  • A detailed and exact inventory of the stolen or damaged property and the amount of the loss in each case, taking into account the original value of such property.
  • Preform invoices supporting the values of the loss.
  • Quotations or preform invoices of the breakdowns or damages caused to the premises.
  • Photocopy of the identity card of the insured or representative.

Machinery Breakdown

After the notice of loss, within 30 calendar days following the date of loss, the Insured shall submit to the Company the following documents:

  • Refrain from making changes to the damaged objects that could make it difficult or impossible to determine the cause of the loss or the importance of the damage, unless the change is made to lessen the damage or in the public interest.
  • Expert report from qualified personnel or company determining the origin/cause of the loss and the extent of the damage.
  • Proforma invoice of the same or similar machinery to the damaged one, indicating the replacement value of the machine.
  • Quotations or proforma invoices supporting the value of the loss.

Credit Card

After the notice of loss, within 30 calendar days following the date of loss, the Insured shall submit to the Company the following documents:

  • Formal letter of claim from the Bank with all the corresponding details.
  • Claim letter addressed to the Bank by the account holder. Signed by the account holder where he/she rejects the charge reflected in his/her account statement.
  • Copy of the transaction rejection form used by the Bank where the account holder denies having made the transaction(s), signed by the cardholder.
  • A copy of the account holder’s identification card or passport showing the registered signature of the cardholder.
  • Copy of the cancelled credit card of the cardholder (both sides of the card).
  • Copy of the account statement showing the transactions claimed by the account holder.
  • Copy of the vouchers or receipts of the transactions claimed.
  • Complaint to the authorities (OIJ) in case of robbery or assault.
  • Letter or notice of robbery, theft or loss of the credit card sent to the Bank by the account holder, requesting its cancellation.
  • Copy of the account holder’s passport. Document required to verify the entry and exit records in case the transaction(s) are made abroad.

Yacht

The Insured must send to ASSA, within 30 days from the date of loss or damage, the formal written claim, detailing the objects lost or damaged and the amount of the claim, proving his insurable interest and the occurrence of the loss or damage, including receipts, invoices, appraisals or other documents showing such interest and proving the amount claimed.

  • Original of the Act of Protest or Report of the Accident made by the Captain before the competent Maritime Authority.
  • Invoices or proforma invoices describing the estimated value of the repairs.
  • Invoices or vouchers of the expenses incurred in the rescue operation or assistance received.
  • A copy of the Sailing Permit and crew list.
  • Report amplifying the details of the accident signed by the Captain and owner of the vessel.
  • Authenticated copy of the Captain’s license.
  • Authenticated copy of the Permit, Patent, or Navigation License of the vessel.
  • Copy of the identity cards of the owner of the ship and Captain.

In case of total loss of the vessel:

  • Original Certification from the Public Registry, indicating the cancellation of the Property Registry.
  • If it is a fishing vessel, a Certification issued by the Competent Authority, canceling the fishing license must be added.
